Academic Success Center Questionnaire

CGTC Institutional Review Statement:

CGTC often conducts surveys utilizing its student body, faculty and staff, following the College’s existing discrimination policy as found in the current catalog. Also, such surveying is conducted in an ethical manner and does not violate the rights, especially privacy rights, of participants nor does it have adverse effects on the health or well-being of those participating.  Demographic data on respondents is gathered collectively and reported in that manner.  Prior consent is gained in all cases of survey research through full disclosure of the intent of each project and either by numerical identifier, verbal agreement, or participant’s signature.  CGTC utilizes its Office of Institutional Effectiveness to review internal and external surveys developed for deployment to personnel and students. 

You have been asked to participate in a survey of the Academic Success Center services.  By entering your student ID, you agree that CGTC may collect your responses and use them in the collection of data to improve services offered at the institution in an effort to better serve students, faculty and staff.
